Crestwood Pharmacy opened its doors in March 2005. Our founder, Dr. Jagat “Jay” Patel, envisioned merging traditional values with modern technology to deliver a truly patient-centered pharmacy experience.

ABOUT JAGAT “JAY” PATEL, PHARMD
Jay Patel considers Albany his home. He graduated from Colonie Central High School, continued his studies at Hudson Valley Community College, and earned a Bachelor of Science in Biology from SUNY Albany. After gaining several years of work experience, he pursued a career in pharmacy and graduated with a Doctor of Pharmacy (PharmD) degree from Albany College of Pharmacy in 2004. Since then, Jay has practiced in clinical, institutional, and long-term care settings, building a well-rounded background in the healthcare field.
In 2012, Jay founded Crestwood Pharmacy, a privately owned and operated business that has since become a cornerstone of the local community. As both the supervising pharmacist and owner, Jay is deeply involved in the day-to-day operations and committed to delivering optimal care and service. Under his leadership, Crestwood Pharmacy has maintained a strong reputation for personalized attention, trust, and accessibility—qualities that have earned Jay the respect of both his peers and patients.

Our knowledgeable pharmacists make sure you receive the correct medication at the time you need it. For your convenience, we offer to our customer’s online prescription ordering as well as online refills. You can also transfer a prescription to us. It’s as easy as 1-2-3!
Always upholding the “Oath of a Pharmacist,” our pharmacists will check for medication interactions and notify your healthcare provider if we find any. We want to make sure that you are using the correct medications at the right dosages. Patients are always treated with respect, and we keep your information private.
Our prescription prices are very competitive. Plus, whatever we don't have, we will order for you to pick up the next day. We also offer a wide selection of over-the-counter medications for your convenience.
